Surgery
All surgery requires a set of medical tools, typically found in the Operating Theatre, along with an operating table.

Surgery Speed and Failure
While every operation has an intended tool, you are not limited to it alone. Every operation has a list of alternative tools you can use instead. Beside each tool is a multiplier which indicates how much of a penalty to speed you are given for using the supplied tool.
These multipliers stack multiplicatively with those from operating site or chemicals (see Beginning of any Operation).
Note, speed penalty is capped to 2x the base time the step would have taken. A step that takes 10 seconds would cap at 20 seconds, for example.
However, every second after that cap will instead add 1 percentage point into the odds of a failure. Failing may cause extra damage or other effects.
So for example:
- Say a surgery step takes 10 seconds by default, but you use a pen (2.5x) instead of a hemostat and do it on the floor (2x) instead of a surgery table.
- Now the surgery is slowed to 50 seconds, but it's capped to 20 seconds.
- Those 30 extra seconds that were removed is now your chance to fail the operation.
- Result: The step takes 20 seconds and has a 30% chance to fail.
The Operation Process
Step 1: Undress the part of the patient that needs surgery. You can do this by dragging them onto yourself, and then clicking the clothes you want to remove from them in the pop-up. Alternatively, some clothes like jumpsuits can be adjusted out of the way, to save yourself some time.
- Important: The uniform the patient is wearing usually covers both legs, arms, groin and torso. Helmets, eyewear and masks always obscure the eyes, make sure to remove them if working on the eyes.
Step 2: Operating Table Place the patient onto the operating table. Do this doing a Hold CTRL and left-click a person with your hand empty. It doesn't matter if Combat Mode Enable with 4, disable with 1 or toggle with F by default. Left-clicking people with an empty hand will Harm them if on, or Help them if off. Prevents you from switching places or being pushed when colliding with people. is on or off. - If there is no operating table, you can use: Stasis bed (1.15x), any table (1.25x), any bed (1.5x), ask them to lie on the floor (2x). The speed of completing every step will be reduced.
Step 2.1 (Optional) Splash on, spray or apply a patch with a reagent like Miner's Salve (0.9x), Sterilizine 0.8x), alcoholic drinks (up to ~0.5x*), or Honey (0.6x) for faster surgery speed.
- The stronger the alcohol, the more effective! The formula is: (booze power / 650). For example: Ethanol gives ~0.9x surgery speed, Manly Dorf ~0.85x, Bacchus' Blessing ~0.5x.
Step 2.2: (Optional) Use a tank of anesthetic and a breath mask to make your patient sleep. This way patient won't experience agonizing pain, decreasing its mood.
Step 3: Place drapes or bedsheet over the part of body you are operating.
- If you done step 1 wrong, it will tell you that you must remove clothing first. If you continue without fixing, you will stab your patient instead of doing surgery.

Step 5: Aim at the appropriate body part (important!).

To get to the operation you want to complete, it will require you get the patient to the necessary state*. For example, an overwhelming majority of surgeries will require the patient's skin be opened first. To open the skin you'll need to complete "Retract skin" first, and to be able to complete "Retract skin", you will need to "Make skin incision". More complicated surgeries require more complicated states, such as sawing bones.
While a patient's undergoing surgery, they will slowly lose blood. Try not to dawdle!
- Note: Some patients you receive will be in a state already ready for suregry! Species like Plasmamen have no skin, so you have nothing to make an incision in. You can simply skip that step!
Step 7: Complete your surgery
After getting to the state required, you can do whatever operation you intended. Congratulations, you just did surgery! That wasn't so bad, right?
Step 7.5: Repeat!
Feel free to do any other operation while the patient is cut open. States don't automatically get cleared after you're done! You can save yourself a lot of time this way.
Step 8: Close the patient
After all is said and done, don't forget to cauterize or suture the operated bodypart! The patient will continue to bleed if you don't.
And you're done! Send the patient out with a lollipop and a smile.
Operations
What follows is a full list of every operation that you can undergo.
Green operations are mechanical surgeries and will only work on cybernetic patients.
Purple operations are locked - typically behind technology from science, but sometimes behind rare or traitorous surgical disks. They cannot be undergone without first being loaded into an operating computer.
